Medical Records Coding

SRL provides a full range of coding services. Services for  inpatient, outpatient, ambulatory, and all specialty medical office coding which includes: ICD-9-CM, CPT, E&M, Coding Audits, Education & Training.

●  Interpret and apply American Hospital Association (AHA) Coding guidelines to articulate and support principle and secondary diagnoses and selected procedures. 

●  Identify and analyze patterns in possible coding errors or other trends and reports to the Supervising Medical Records Coder. Participates in mandated Medical Record Review processes. 

● Using the current HCPCS and ICD-9-CM and CPT coding systems, assigns and records an accurate code to all diagnoses, procedures, and operations as documented by the attending physician in the indicated patient's medical record. 

● Insure all factors necessary for assigning an accurate DRG (Diagnostic Related Group) are present, and all diagnoses are ranked properly. 

●  Make appropriate contacts in order to acquire or clarify necessary information. 

●  Entered final diagnostic code numbers and narrative descriptions into grouper system. 

●  Compile and update the appeal log detailing denials, hospital's reply and follow-up responses. 

●  Provide information and respond to inquiries regarding medical documentation and DRG's to hospital staff  including Utilization and Quality Assurance staff, Patient Accounts staff and the Risk Manager. 

●  Our solutions encompass cost reduction, utilization improvement, and revenue improvements. 

Medical Records Coding Training 

The US Department of Health and Human Services (HHS) has mandated the replacement of the ICD-9-CM code sets used by medical coders and billers to report health care diagnoses and procedures with ICD-10 codes, effective Oct. 1, 2015. ICD-10 implementation will radically change the way coding is currently done and will require a significant effort to implement.
